Killeen Daily Herald: ‘Former platoon sergeant was on leave during the [Fort Hood] rollover accident, yet found at fault’ (June 2, 2018)

“The whole thing was hard to process. I was angry, I was pissed off, and I didn’t know how they were going to charge me,” Caroline Blaze said. “All I knew was I was being charged and I was literally done with the Army.”

Blaze said she received a local general reprimand that stayed in her records for a year around the same time she was being processed for a medical retirement. She said she was never notified or saw the recommendation for her punishment and to this day she doesn’t know what was written about her.

Blaze said she even became suicidal and had a suicide attempt several months after receiving the reprimand.

“It was a bad time and a hard time. To say that I was at fault for these soldiers lives like I caused their death.”

Fact: In the last two years, more soldiers at Fort Hood died stateside than overseas. The six non combat deaths that occurred overseas were insider attacks (4) and death ruled suicide by the Army CID (2). The following numbers are the specifics at Fort Hood in Texas since January 2016.

78 deaths at Fort Hood since January 1, 2016. 

  • As of August 2017, 9,300 soldiers from Fort Hood were currently deployed across the globe, this is more than a 1/4 of the 35,000 troops stationed there
  • Average age of death is 28 years old
  • Average 1.5 suspected suicides per month since January 2016
  • 7 overseas deaths to include 4 insider attacks and 3 non combat deaths
  • 71 stateside deaths to include 37 alleged suicides (13 soldiers had no known deployments; 24 soldiers had deployed to Korea, Iraq, or Afghanistan)

Navy sailor Andrea Daves’ deceased body was found in a charred vehicle at the end of a cul-de-sac in Lakeside, California on February 22, 2017. She was a mother of two children and on maternity leave at the time of her death. Initially the death was ruled as a homicide but in May 2017, the San Diego County Sheriff’s Homicide Unit ruled the cause of death as suicide due to thermal burns.

“Her mother, Pamela Johnson, told 10 News in February that her daughter loved serving in the Navy and was a good person. Johnson believed her daughter had been killed by someone else.” –East County Magazine

Fort Hood Soldier Pvt. Michael Garcia, 29, died of injuries suffered during a vehicle training incident at the National Training Center at Fort Irwin, California on February 17, 2017. Pvt. Garcia’s home of record is listed as Fayetteville, North Carolina and he joined the Army in March 2016. Pvt. Garcia was an indirect fire infantryman assigned to the 4th Squadron, 9th Cavalry Regiment, 2nd Armored Brigade Combat Team, 1st Cavalry Division at Fort Hood, Texas. The circumstances surrounding the incident are under investigation by the Army Criminal Investigation Division (CID).

Army Green Beret Warrant Officer 1 Shawn Thomas, 35, died February 2, 2017 in a vehicle roll-over accident that occurred during a routine administrative movement between partner force outposts in Niger, Africa. WO1 Thomas was conducting advising operations in Niger on behalf of the 1st Battalion, 3rd Special Forces Group at Fort Bragg, North Carolina. WO1 Thomas had seven combined deployments to Iraq and Afghanistan. Shawn’s wife met his casket at the airport and the moments she had with him were captured on video.

Army Spc. Barron Von Reichelt, 24, died from injuries suffered in an automobile accident on South Range Road at Fort Hood on January 7, 2017. Spc. Reichelt’s home of record is listed as Las Vegas, Nevada; he joined the Army in January 2015 as a cannon crew member. Spc. Reichelt was assigned to the 2nd Battalion, 82nd Field Artillery Regiment, 3rd Armored Brigade Combat Team, 1st Cavalry Division at Fort Hood since April 2016.

Army Specialist Ronald Murray, Jr., 23, died of a non combat related injury in Kuwait on November 10, 2016. Specialist Murray was supporting Operation Inherent Resolve on behalf of the 4th Battalion, 1st Field Artillery Regiment, 3rd Armored Brigade Combat Team, 1st Armored Division in Fort Bliss, Texas. Specialist Murray worked in fire support and died in a vehicle accident. He is from Bowie, Maryland and joined the Army in January 2015.

Fort Hood Army Pvt. Dakota Stump Found Dead on Post Three Weeks After Vehicle Accident; Family Wants Missing ‘Warrior Alert’ Law (2016)

Pvt. Dakota Stump, 19, US Army, went missing from Fort Hood, Texas on October 10, 2016. The family confirmed Dakota was found dead on November 3rd, 2016 on the base. Military officials said Stump’s remains were found next to his flipped-over vehicle 100 yards from the roadway on Fort Hood. An investigator told Dakota’s mother Patrice Wise that his car had rolled multiple times and he was ejected but the crash scene wasn’t visible from the road. Patrice claimed Army officials said they searched the area but because his phone pinged off a cell phone tower in Indiana they thought he was Absent Without Leave (AWOL). We need to change Standard Operating Procedures on how Law Enforcement and Military handle situations when our Veterans and Active Duty Members go missing. Which will in turn allow not only Law Enforcement and Military Officials the tools to better locate these individuals, but will help the families and any veterans organizations to assist as well. –Dakota’s Law Petition

1st Lt Jeffrey Cooper, US Army, died of a non combat related injury in Kuwait on September 10, 2016. 1st Lt Cooper was supporting Operation Inherent Resolve on behalf of the Headquarters and Headquarters Company, 2nd Battalion, 502nd Infantry Regiment, 2nd Brigade Combat Team in Fort Campbell, Kentucky. According to the Department of Defense, the incident is under investigation.
